School Nurse

The school nurse is available on a daily basis for assessment and counseling regarding health issues.  It is important that students come during non-academic times, except in an emergency situation.  Students are required to have a pass from the teacher of the period in which they are visiting the nurse.  Students who become ill during the school day must see the nurse for dismissal.  The nurse will make arrangements with a parent or emergency contact and a dismissal pass will be issued.  The pass then needs to be stamped by the SAO office before the student leaves school.  All accidents occurring at school must be reported to the teacher and the nurse before leaving school.

The Nurse's office is open Monday thru Friday 7:30-1:30, 2:05-2:50, and by appointment.
The Nurse Practitioner is onsite Mon and Tues 7:30 - 2:30, and Thurs 7:30 - 1:30

Medications at school

  • All medication, prescription and non-prescription alike, must be kept in the Student Health Center. Written permission from the doctor and a parent/guardian is required for the nurse or her staff to administer medications. A medication permission form is available as a pdf file.  Medications must be sent to school in the original bottle. For prescription medications the prescription bottle may serve as a doctor's order. If a student is on long-term prescription medication that is to be administered by the Student Health Center staff, the prescribing doctor must complete a Permission to Administer Medication form.

Mission:
  • To provide access to urgent medical care, preventive and behavioral health services to students who would otherwise not receive care.
 
Goals:
  • To join MSAD #75 in its dedication to develop confident, healthy life-long learners.
  • To emphasize the role health and fitness play in confidence, interest, energy and ability to learn.
  • Enable students to practice behaviors that lead to personal wellness.
How:
  • This program is in addition to the traditional school nurse services. As always, there are no charges associated with school nurse visits. The Nurse Practitioner and the school nurse work closely together to provide the best service to students.
  • The School Based Health Center is located in the Nurse's office, and is staffed by an administrative assistant. The Nurse Practitioner who sees patients three days per week, and is also available by phone if circumstances warrant.
  • Enrollment forms and information release forms are sent home on the first day of school. We accept new members throughout the school year. Forms are available in the School Based Health Center, nurse's office, SAO, Guidance department, and online, below.
  • Appointments can be made directly with the administrative assistant or through the school nurse. Every effort will be made to schedule all appointments during study halls or during a time that least impacts the student's schedule.
  • Cooperative contract with Keri Chaput, LCSW to provide all mental health services. She will require separate forms for consent and insurance information. They do bill all insurance and may require co-pays as required by your insurance plan.
Services:
  • Diagnose and treat acute illnesses (sore throat, bronchitis, injuries, rashes etc.)
  • On site lab tests for strep throat, urine tests, anemia
  • Referral to hospital for lab and X-ray services
  • Management of chronic conditions (asthma, diabetes, headaches, acne etc.)
  • School sports and camp physicals
  • Coordination of care with student¹s primary care provider
  • Referral to specialists as needed
  • Assistance with tobacco cessation/ quit smoking classes available
  • Alcohol and substance abuse referrals
  • Counseling services provided by Keri Chaput, LCSW
  • Assistance in acquiring MaineCare
  • Assistance in finding a primary care provider if needed
Eligibility:
  • With permission of parent or guardian, students of Mt. Ararat High School . Staff are welcome by appointment.
Hours:
  • The Nurse Practitioner is available on:
    Monday 10 a.m. - 2:15 p.m.
    Tuesday 8 a.m. - 1:15 p.m
    Thursday 8 a.m. - noon
    or by appointment.
Payment / Billing
  • The is no fee for the health office. .
  • MaineCare will be billed for services at the health center. Certain Anthem, Aetna, and Cigna plans may be billed for services this year under a pilot program negotiated with the insurance companies. No referrals or co pays are needed.
Additional Information
Parents/Guardians are very welcome to call the SBHC or the School Nurse for any questions or concerns regarding their child's health. Maine State Laws regarding parental consent for care are the same as they are at your doctor's office; parental consent is not required for the treatment of drug abuse, pregnancy testing and testing for and treating sexually transmitted diseases.
